Patient Advocate Foundation's Co-Pay Relief Program

The Patient Advocate Foundation (PAF) Co-Pay Relief Program (CPR) currently provides direct financial support to insured patients, including Medicare Part D beneficiaries, who are financially and medically qualified for pharmaceutical treatments and/or prescription medication co-payments, co-insurance and deductibles relative to your diagnosis.

Eligibility Requirements

  • Patient should be insured and insurance must cover the medication for which patient seeks assistance
  • Patient must have a confirmed diagnosis of the disease/illness for which they seek financial assistance
  • Patient must reside and receive treatment in the United States
  • Patient's income must fall below 400% of the Federal Poverty Guideline (FPG) with consideration of the Cost of Living Index (COLI) and the number in the household

 

Online Application Portal for Patients, Providers and Pharmacies

The CPR Program offers four points of entry:

  • Patients may apply via the Patient Online Application Portal available 24 hours a day
  • Medical Providers may apply on behalf of their patients via the Provider Online Application Portal available 24 hours a day
  • Pharmacies may apply on behalf of their patients via the Pharmacy Online Application Portal available 24 hours a day
  • The program offers personal service to all patients through the use of an Application Specialist, personally guiding patients through the enrollment process toll free at 866-512-3861, Option 1
